I designed the health folders to be a coloured manilla folder containing pre-printed forms from me. I buy the folders from a stationary supplier in bulk (unisex colour-orange/green/yellow) bright enough to stand out so they can locate it easily.
On the outside you staple your business card and a card sized piece of paper with your consultation hours and other arrangements, e.g. picking up of remedies. Inside, on the left hand side, I attach the women's or men's medical health check form ( men's form is available free- go home page), so it is a quick reference for all the medical/pathology tests the patient has had done, or needs to do. These tests are designed for the early detection and prevention of major, common diseases.
